PHOTO AND RECORDING PERMISSION FORM

Dear Parent/Guardian: Seattle Women's Jazz Orchestra is a nonprofit jazz orchestra that wishes to produce and distribute one or more recordings, in any media or format, containing certain performances of the Seattle Women's Jazz Orchestra and its guest performers, the Girls Jazz Band.

We will take pictures and may record audio and video of the Girls Jazz Band program participants and events to document, archive, and promote this educational service. Rehearsal and concert recordings are an integral part of the music education process and for evaluating the program effectiveness.

We would like your permission to use your child's picture in publicity materials to assist in sustaining this new educational program. The pictures and recordings may be used for a variety of publicity to include, but not limited to, recruiting and enrollment forms, newspapers, press releases, grant applications, and grant final evaluation reporting, etc.

Because each participating girl is part of the larger band, it would be impossible to record the concert without permission from all participants. As such, signed parent/guardian permission is required for all student participants.

Please use PayPal for the $60 USD registration fee
